Pre-Program Questionnaire (PPQ):

No one can compartmentalize the stresses from home life and work life. Stress results in absenteeism, presenteeism and accidents, addiction, illness, co-worker conflict, and more. All of these cost your company in lost productivity, increased overhead, and lost sales.
Our PPQ can be administered in hardcopy or online. All results asre all results are analyzed by Program Coordinator before being released - not just calculated by software. It is best administered at the start of a wellness program, or when you are considering large changes to the one you have in place.
When you need a realistic view of what is happening in your organization - this is the questionnaire for you. You may even be shocked by some of the results; the questions include look at coworker conflict, employee engagement, sense of mastery, in addition to attitudes to wellness, and where they are on the Prochaska Stages of Change.
The questionnaire takes approximately 45 minutes to one hour to administer.

There are two reports generated from the questionnaire: one for the employee detailing his or her wellness profile,
and the aggregate data report. The employee profile gives them an honest appraisal of their current lifestyle and
highlights areas where they are doing well and where they might want to focus some effort, with some gentle
suggestions.
The aggregate report for the company does not identify individual employees. Rather it gives an overall picture of
what is happening in the workplace, from a variety of measures including the Gallup Q12 (for employee
engagement) and the Stress-Satisfaction Offset Scores. It also tells you the interests of the participants as a whole,
identifying which wellness practices are more likely to be warmly received by the employees and which will not
entice the participation you would expect.
This report is presented to your senior team by our Program Coordinator when the Questionnaire is administered as
part of an on-going process.
